Contents
Paracetamol 500 mg, caffeine 65 mg
Indications/Uses
Relief of mild to moderate pain including headache, migraine, backache, muscular aches, menstrual cramps, arthritis pain, toothache & pain associated w/ common cold & flu. Reduction of fever.
Dosage/Direction for Use
Adult & childn ≥12 yr Pain &/or fever 1-2 tab every 6 hr or as needed. Max: 8 tab/24 hr.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. High BP or irregular heartbeat. Long-term use w/ anemia or heart, lung, kidney or liver disease. Patients recovering from severe alcoholism & taking disulfiram. Drugs for high BP (eg, propranolol, atenolol), caffeine & depression or anxiety (eg, sertraline, clozapine), lithium carbonate, decongestants containing ephedrine, theophylline, levothyroxine. Childn <12 yr.
Special Precautions
Severe liver damage in large doses, other drugs containing paracetamol & ≥3 alcoholic drinks daily. Limit taking caffeine-containing drugs, food (eg, chocolate) or beverages (eg, coffee, tea, cola). Kidney or liver problems. Patients taking warfarin. Discontinue use if new symptoms occur, symptoms & headache persists, pain worsens or lasts for >10 days, fever worsens or lasts for >3 days. Pregnancy & lactation.
Adverse Reactions
Paracetamol: Hypersensitivity including skin rashes, minor stomach & intestinal disturbances. Caffeine: Tremor, difficulty in sleeping, nervousness, restlessness, irritability, anxiety, headache, ringing in the ears, fast or irregular heartbeat, rapid breathing, frequent urination & stomach upset, increased BP. W/drawal symptoms eg, headache, tiredness & decreased alertness.
Drug Interactions
Paracetamol: Accelerated absorption w/ metoclopramide or domperidone. Reduced absorption w/ cholestyramine. Increased susceptibility to harmful liver effects w/ drugs for convulsion (eg, phenobarb, phenytoin, carbamazepine). Increased in INR w/ warfarin. Increased risk of liver toxicity w/ alcohol (eg, ethanol) or INH. Possibility of severe hypothermia w/ phenothiazine. Decreased clearance & increased effects w/ probenecid pre-treatment. Caffeine: Weakened vasodilating effect of substances used for myocardial imaging (eg, adenosine, dipyridamole). Antagonistic effect w/ sedatives & tranquilizers (eg, diazepam, zolpidem). May increase action of phenylpropanolamine in increasing heart rate. Competitively inhibits clozapine metabolism. Slight to moderate increase in serum lithium carbonate levels. Increased stimulant effects w/ MAOIs (eg, phenelzine, moclobemide, selegiline). Reduced clearance & increased effects w/ methoxsalen & pipemidic acid. Increased BP w/ levothyroxine. Increased clearance & decreased effects w/ phenytoin. Increased blood sugar & lactate conc w/ ephedrine.
